Federal Highway Administration's Report

The Arizona Department of Transportation (ADOT) is asking for municipalities to fill out the Federal Highway Administrations form. ADOT is responsible for compiling and submitting the biennial local highway finance report often referred to as the Federal Highway Administration's form 536 (FHWA Form 536). This report helps the FHWA determine revenue and expenditure trends by local governments that is essential in policy and program development. FHWA Form 536 is an important source of data for federal, state, regional, and local transportation officials. Without your cooperation, this valuable information cannot be accurately and reliably compiled. Once the information is received from the cities, towns and counties, we will compile and submit it to the FHWA.

The FHWA Form 536 should be completed by December 16, 2011 and emailed to Phil Chan at ychan@azdot.gov. If electronic submission is not possible, the completed form may be mailed to him at 206 South 17th Avenue, Room 200B, Phoenix, AZ 85007, or faxed to him at (602) 712-6672.
